ClearToDrive Consultation Agreement

By proceeding with payment, you acknowledge and agree to the following:

• ClearToDrive provides consultation services only to help you prepare for your DOT physical examination.
• ClearToDrive does not perform DOT physical exams and is not affiliated with the medical examiner or clinic that will evaluate you.
• We do not guarantee you will pass your DOT physical exam. Final determination is made solely by the certified medical examiner.
• Our goal is to help you avoid delays or repeat visits by ensuring you are properly prepared; however, additional testing, documentation, or follow-up visits may still be required.
• Payment is for consultation services only and is non-refundable once the consultation has been provided.
